UNITY, NH 03743 TEL: (603) 542-9511 EXT 210 FAX: (603) 542-2829 Email: sherriec@sullivancountynh.gov The Department of Human Services is responsible for recipients in nursing homes and services for the elderly or chronically ill residing in their own home or at mid-level care. With the implementation of House Bill 2 effective July 1, 2008 the counties are responsible for long-term care programs, Nursing Home Care (INC) and Home and Community Based Care (HCBC). The county pays 100% liability for the non-federal share of these two programs. INC funds nursing home care for Medicaid-eligible recipients in private nursing homes as well as county nursing homes. The elderly are encouraged to remain in their homes or in a setting less intense than a nursing home. Medicaid eligible elderly can opt to have services in their own homes through the HCBC program. Counties have received prevention funds from the State of NH Division for Children, Youth and Families, known as the 6% Incentive Funds. These funds were established with the passing of Senate Bill 1 in 1986. The Administrative rules are being re-written by DHHS and will eliminate the counties from the receipt and administration of this funding.
