The Sullivan County Human Services department serves the elderly, the disabled, and the youth and families throughout our county that qualify for state mandated services.
For adults receiving money payment: Old Age Assistance or Aid to the Permanently and Totally Disabled, the cost is shared 50/50 with the state.
Residents residing in Nursing Homes, eligible for Medicaid the program cost is split three ways: Federal 50% County/State 25%.
Legislation (SB 409) passed and was implemented January 1, 1999, which reduced the portion of the county obligation for nursing home costs from 30.55% to 25%. With the new legislation the counties became responsible for two new programs, Provider Payments for a menu of medical services for Medicaid eligible residents residing in a nursing home. Also, Home and Community Based Care (HCBC) medical services are provided to those that are medically eligible to enter a nursing home but wish to remain in their home. The county is responsible for 25% of the costs of these two programs.
The County shares the cost of services delivered to the children & families with the Division of Children, Youth & families with a split of 75% State and 25% County. These services are court ordered and may include such services as Foster Home Care, Group Homes, counseling, legal services, home based services, respite, etc. The County responsibility is to review each bill for court ordered services for accuracy and to authorize payment by the State of New Hampshire.
The County is also responsible for collecting Parental Reimbursement from parents of children receiving court ordered services.
The County receives money from the Division Of Children, Youth and Families annually for programs implemented to keep children from out-of-home placement. An application process is used and the money is awarded through a review panel.
