One of the most difficult decisions for any family is choosing an appropriate nursing home for a loved one. You want assurance that your family member will be receiving good medical care and an opportunity to continue their lives with true meaning and quality. Sullivan County Health Care, located on County Farm Road in Unity, New Hampshire, was designed to provide comfort, safety and a home-like atmosphere for our residents. Our team of qualified professionals provide 24-hour nursing care with personalized attention to each resident. Our Rehabilitation Department has on staff a registered Physical Therapist, as well as a registered Occupational Therapist, when needed. Our Activities Department works closely to ensure that your loved one’s activities and recreational needs are geared to their interests and abilities. We are a Medicare approved Skilled Nursing Facility (SNF). If your doctor determines that Skilled nursing placement is appropriate, following a qualifying stay in the hospital, we are able to accommodate your needs. In 1997, our MacConnell Unit was opened. This wing was built specifically to meet the needs of the resident with Alzheimer’s disease. It is a safe and secure environment to provide the specialized rehabilitation and activity programs for these special residents. Sullivan County Health Care can help you with Respite Care needs. If you need to take some time off from caring for your loved one at home, we can admit your family member for whatever length of time is needed and you can be assured they will receive very good care while in our home. We are a New Hampshire Medicaid participant and can assist you in applying for Nursing Home assistance when or if needed. The goal of the entire staff is to personally care for and professionally serve each of our residents.
